The purpose of this form is to enable the students to provide the Research Graduate School with confidential feedback about their progress.

Before completing this form we recommend that you read through all of the questions and type your free text answer to Question 9 in a Word document (and check the word count) first so that you can copy and paste itin to the form.

This will ensure that should you fail to correctly complete each question before submitting the form you will not have to retype your answer if the form resets.
